Project Overview

We have previously shown the benefit of RPM for patients with Hypertension (HTN) and Diabetes (DM).In this project we will demonstrate value of RPM for patients high risk for readmission because of chronic conditions like Diabetes and Congestive Heart Failure. We will measure impact on ED visit, hospitalizations and healthcare spending.
