Project Overview

The goal of this work plan is to analyze clinical data from the Remote Patient Monitoring Program for Hypertension Management including program implementation, clinical outcomes, and comparison to routine care. Furthermore, using lessons learned and data analytics, we will use this information to inform continued growth and medical oversight of the existing Remote Patient Monitoring clinical program for chronic disease management including hypertension, diabetes mellitus, and congestive heart failure. We will track ongoing enrollment, growth, and clinical sustainability of these programs.
