Project Overview

CARES @ The Navigation Center (TNC) is a telehealth-based free clinic that provides primary care at a drop-in resource center for PEH in Charleston. Additionally, House Calls for the homeless (HCTH) is a newly developed ‘street medicine’ program that provides medical care and other supportive services to unsheltered homeless individuals as part of an interprofessional team. As the hours during which synchronous telehealth services are available limits their accessibility for a transient patient population with many competing priorities, we will introduce primary care e-visits at both CARES @TNC and HCTH.
