Project Overview

This project will develop, implement, and evaluate a model for reimbursement for delivering the National Diabetes Prevention Program (DPP) in a telehealth setting with 360 Medicaid patients at risk for type 2 diabetes (18 cohorts). Specifically, we propose to (1) develop a value-based care fee structure for Medicaid reimbursement for the DPP; (2) conduct an analysis of medical expenditures among Medicaid patients with risk for diabetes who participate in the telehealth DPP to measure net savings and return-on-investment (ROI) relative to control matched non-participants; and (3) conduct a longitudinal cohort analysis to assess the incidence of type 2 diabetes in DPP compared to non-DPP control matched patients.
