Project Overview

The Centers for Disease Control and Prevention’s (CDC) Division for Heart Disease and Stroke Prevention (DHDSP) supports state and local health departments in implementing strategies that improve Cardiovascular disease (CVD) outcomes for the populations they serve. In the summer and fall of 2021, CDC conducted a systematic screening and assessment (SSA) and solicited nominations from almost 200 organizations throughout the U.S. UMMC’s RPM program was one of the three telehealth strategies selected for the rigorous evaluation.
